Completing the pre-admission process prior to your admission date will assist in making your admission on the day as streamlined and hassle free as possible.
For your convenience, there are two methods in which you can submit your pre-admission documentation:
Pre-Admission at the hospital
We encourage you to visit the pre-admission nurse or clerk based at the hospital. They will be able to guide you through the process and assist you with the completion of the documentation.
Download and Email your documents
Should you wish to complete and submit the documents at your own convenience, you are able to download the pre-admission form from the website and submit the completed form with the necessary support documents to the email address detailed below. We kindly request that this is sent 48 hours prior to your admission to ensure that there is enough time to obtain your pre-authorisation number from the medical aid in advance.
Please email your completed form, ID, Medical Aid Card copy and your doctor’s referral letter for admission.
